By providing my phone number and responding YES to the first text message from Sierra Foot & Ankle, I consent to receive SMS text messages from Sierra Foot & Ankle for appointment reminders, follow-up care instructions, clinical communication, office updates

By replying YES, I acknowledge:

1. Texting is not secure. Sierra Foot & Ankle will NEVER ask for protected health information. Text IS our main communication channel with patients for the purposes listed above.

2. Consent for texts is not required for healthcare services.

3. Consumer information is not shared with third-parties for marketing purposes.

4. This consent applies to the mobile number receiving the SMS message.

2. Message Frequency
You will receive up to 4 messages per month dependent upon your communication with us.
3. Message and Data Rates
Message and data rates may apply based on your mobile carrier’s terms.
4. Privacy Policy
Your information will be handled in accordance with our Privacy Policy, which can be

5. Opt-Out Instructions
