Health care is a journey that requires communication, trust, and teamwork between you and your medical provider, so it’s important to us that we start our relationship with one another on the right foot (pun intended).
We understand that your time is valuable, and your health is critically important, so we work hard to make sure each office visit is comfortable, efficient, and productive. The information and documents on this page are here to help you know what to expect, and prepare for your visit, so everything goes as smoothly as possible.

The first visit is more than a first impression. There’s a lot of background information we need about you—not just your medical history, but also your personal goals, preferences, and lifestyle. This information provides the essential foundation that allows us to tailor treatment protocols to best suit your needs.
The first visit is also a great opportunity for you to get to know us better, too. Please feel free to ask us anything! We want to give you every opportunity to have all of your questions and concerns heard and discussed.
Filling out new patient intake forms does take time and can be stressful—especially when you’re trying to complete them in the waiting room before your appointment! The initial paperwork can take 15-20 minutes to fully complete.
To make the process as easy as possible, we now allow you to complete and submit your new patient registration and medical history paperwork 100% online—no need to get pen and paper involved! (Of course, you can still print a blank copy of your form and bring it with you if that’s what you’d prefer to do.)

Please bring the following items so you can check-in efficiently:
- Photo ID (such as driver’s license)
- Insurance cards
- Medication list (if applicable)
- Completed New Patient paperwork (hyperlink above)
Insurance and Payment
We do participate with many major health care carriers, including HMO and PPO plans, but unfortunately not all plans cover all services.
If you have any concerns that your insurance may not be accepted, please contact us ahead of time for verification. You will be expected to handle all costs not covered by your plan, as well as all co-pays due at the time of service. If we are not able to verify your insurance coverage ahead of your office visit your appointment will treated on a cash basis.
